Notes into four sections

The default pass on notes you took while half-listening.

Turn these notes into four short sections: decided, actions with owner and date, open questions, and anything raised then dropped. Discard discussion that went nowhere. If the meeting produced no decisions at all, make that the first line rather than padding the sections.
